1. To create a Treatment Plan, from CP Central select the Client Menu.

2. Select the Client Details icon (single person) to navigate to the client profile.

3. From Client Details select the Clinical Tab.

4. Scroll down to the Treatment Plan section. If there is already an Active Treatment Plan and the desire is to continue with that Treatment plan, select the Clone icon (two pieces of paper).

5. Or if the desire is to create a new Treatment Plan, select the Add icon (plus sign).

6. On Step 1 - General complete the various fields.

7. When finished, select Next at the bottom.

8. On Step - 2 Diagnoses Goals/Interventions enter the Treatment Frequency for the first diagnosis. Then select Add/Edit Goals.

9. Select +Add new record.

10. Enter the goal. If additional goals are needed for the first diagnosis, select +Add new record again. When finished, select Save & Exit.

11. Target Dates, Statuses, and Objectives may now be added for the goals. To create and intervention, select the edit icon (pencil) next to Interventions.

12. Similar to Goals, select +Add new record to add an intervention.

13. Enter the intervention.

14. Select Staff, if applicable.

15. Select Location, if applicable.

16. To add more than one intervention, select +add new record. Select Save & Exit when finished.

17. If more than one diagnosis exists, select the additional diagnosis and complete steps 8-16. When finished, select Next in the bottom left corner.

18. On Step 3 - Review, review the information entered for accuracy. After reviewing the client will need to sign.
There are two options for obtaining the client signature. If the client is present in-person, they can write in the blank box, and then select "Sign as....". If the client is not present, select Request Signature Instead and the Treatment Plan will be sent to the client via the Client Portal. Note- The Client Portal will need to be enabled to utilize this option.

19. Once the client has signed, the staff signature needs to be applied. Select Sign and Forward to Sign the Treatment Plan and forward to the reviewer at the same time. If staff are their own reviewer, select Apply Stored Signature and then select Review under the Review section.

20. When done, select Finish and then Ok on the pop up.

21. On the Clinical tab under the Treatment Plan section the Status of the Treatment Plan can be viewed. The status will depend on the signatures needed. If all signatures (client, staff, reviewer) are applied the status will be Active.
