1. From CP Central, select the Client Menu.

2. Select Client Details (single person icon) of the client you wish to create the Treatment Plan for.

3. On the right side of the screen under the section "Action Center" ensure the client is registered for the Healing Hope Portal.

4. Next, select the Clinical tab to write the Treatment Plan.

5. Scroll down to the "All Plans" section and select the plus sign to start a Treatment Plan. If a Treatment Plan is already started, select the edit icon (pencil) of the "Inactive" Treatment Plan to finish and obtain signature.

6. Complete Step 1 - General" of the Treatment Plan.

7. When finished, select "Next" to go to "Step 2 - Diagnoses Goals/Intervention".

8. Enter the "Treatment Frequency".

9. Select "Add/Edit Goals" to enter the first goal.

10. A pop up will appear, select "+Add new record".

11. Enter the goal in the text box and then select "Save & Exit". If multiple goals are needed for the first Dx, select "Add new record" before "Save & Exit".

12. Enter the "Objectives".

13. Select the edit icon (pencil) to add an intervention for the goals(s).

14. A pop up will appear, select "+Add new record".

15. Enter the intervention in the text box, click "Select Staff" and "Select Location", if needed. If multiple interventions are needed for the first goal, select "Add new record".

16. When finished adding interventions select "Save & Exit".

17. After goals, objectives, and interventions are added for each Dx code on the left, select "Next" to go to "Step 3 - Review/Sign".

18. The client will need to sign first either via the Client Portal or in-person. To have the client sign via the portal, select "Request Client Signature" or to sign in-person, select "Sign Here".

19. If "Request Client's Signature" is selected a notification will appear stating it has been sent to the client via the portal. The client will receive an email notification notifying them a document needs their signature. You may now exit the Treatment Plan.

20. If signature was requested via the Client Portal, after the client has signed the Treatment Plan you will receive an email notification stating your review is needed on a document. Navigate to the client profile and select the "Docs & Dates" tab.

21. Under the section "Shared Documents" click the magnifying glass to review the signature and Treament Plan.

22. A popup will appear to display the Treatment Plan and client signature. When you are finished reviewing, select the "Approve" button at the bottom.

23. If you navigate back to the Treatment Plan via the "Docs & Dates" tab you'll see the Treatment Plan is now active.

24. If "Sign Here" or "Sign Here Instead" is selected a box will appear. The client can then sign in the box and click "Sign as....". Staff signature will then be able to apply their signature.
