1. First step is to create the document. If the document is a word document, you will want to print it to PDF to turn it into a PDF document.
2. To "Print to PDF" select "File" in the word document.

3. Select "Print". Make sure the "Printer" option is set to "Microsoft Print to PDF".

4. Select a place to save the PDF, name it and select "Save".

5. Next step is to make the form fillable. Go to www.sejda.com.
Sejda is a free PDF editor that we use frequently.

6. Select "All Tools" at the top.

7. Select "Create Forms".

8. Select "Upload PDF file".

9. At the top there will be numerous options at the top to create different fields on the form.

10. Select "Text Field" for example and box will appear. You can drag the box to the appropriate location on the form.

11. Continue to edit the fields and select "Apply changes" at the bottom.

12. Select "Download" when finished.
