1. From CP Central, click on the Clients Menu.

2. Select the Client Details icon (single person) for the client you wish to write the assessment on.

3. Select the Clinical Tab.

4. Scroll down until you come to the Assessments and Diagnoses section and then select the plus sign to add the assessment.

5. A popup message will appear with two options. Select "Create Assessment".

6. You'll be directed to the Client Assessment Edit screen. Here you can navigate to the different section on the right to complete the assessment.

7. The last tab to complete is the Summary tab. Here you need to make sure you enter the Assessed date and that your name appears as the staff.

8. Then enter the diagnosis codes.

9. If you need to save the assessment and come back to it, select Save Only or Save & Exit.

10. If you have completed the Assessment, you can then Sign and Forward to your assessment reviewer. If you do not have an assessment reviewer, you can simply click "Review".

11. A popup will appear, select "Sign and Forward".

12. If you forwarded to an assessment reviewer, you would see your signature and the date it was sent to the reviewer.
