1. From CP Central, select the "Clients" menu.

2. Select the Client Details (person) icon.

3. If a Dx is already present, it will be displayed under "Client Basics" and "Action Center". If no Dx is present, select the "Assessments & DXs" tab.

4. Select the Add icon (plus sign) under the "Active Assessments and Diagnoses" section.

5. A popup menu will appear. Select "Add Dx Code Only" on the left.

6. Enter the "Start Date".
It's important to note that the dates inputted for diagnoses will impact both billing and treatment plans. The Billing and Treatment Plans generated will show the diagnosis code corresponding to the session's timeframe and the Treatment Plan.

7. Select the "Type".

8. Select the diagnoses.
The diagnoses populated will depend on the "Type" selected in the previous steps.

9. Continue to add diagnoses codes until all are entered. Select "Save".

10. Next, the diagnoses added will display. If only a diagnosis was added and no corresponding assessment was performed, then the "Type" will display as "Diagnosis Only". Select the arrow to the right to view the added diagnosis code.

11. The staff that added the diagnoses codes will display under "Staff". The "Signature" column will remain blank.

12. To end date a Dx code, simply add another Dx entry and the end date will automatically populate based on the start date of the new entry.
